I agree not to hold Northland Elite Basketball Club, Inc. or any of its coaches, volunteers, agents or representatives accountable for any injuries which may be incurred while participating in the program. If, during the course of my son or daughter’s activities in basketball, he or she should become ill or sustain an injury, I hereby authorize you to obtain emergency medical/dental care. I will assume financial responsibility for the bills incurred through my insurance company. *
